The practice, called locum tenens doctors or nurses, refers to instances where a professional works on a temporary basis when the need is there and moves on to another location when the workload has dropped. This is becoming more common now among dentists. One dentist, <a target="_blank" href="http://www.dentistryiq.com/index/display/article-display/5479827039/articles/dentisryiq/practice-management/2009/12/dentists-taking_to.html" onclick="pageTracker._trackPageview('/outgoing/www.dentistryiq.com/index/display/article-display/5479827039/articles/dentisryiq/practice-management/2009/12/dentists-taking_to.html?referer=');">Dr. Herbert Earnshaw</a>, has found the practice of locum tenens very fulfilling and has used it to replace his regular practice.<br />
<blockquote><span id="ContentBody">Like Dr. Earnshaw, a growing number of dentists are finding opportunities to take their act on the road by working temporary assignments in a variety of settings. The trend is being driven by a national shortage of dentists and mirrors a similar development in medicine, where tens of thousands of MDs work as locum tenens each year. In the field of medicine, private practices, hospitals, community health centers, correctional facilities, military facilities, and other providers are using l<em>ocum tenens</em> physicians to maintain services when permanent physicians can’t be found. This often takes place in federally designated Health Professional Shortage Areas (HPSAs) where the ratio of practitioners to the population falls below the standards set by the Health Resources and Services Administration (HRSA).</span><br /><span id="ContentBody"></span><br /><span id="ContentBody">There are over 6,000 HPSAs in the U.S. today where the number of primary care physicians falls below HRSA standards. At over 4,000, the number of dental HPSAs is not far behind. Companies like Staff Care, which for years specialized in the placement of <em>locum tenens</em> physicians, are being asked to find temporary dentists to fill openings around the country.&nbsp;&nbsp; </span><br /><span id="ContentBody"></span><br /><span id="ContentBody">Dr. Earnshaw, for example, worked as a <em>locum tenens</em> in a private practice, substituting for a doctor who was recovering from hip replacement surgery. He also worked temporary assignments at community health centers and correctional facilities that are chronically short of permanent dentists. Other dentists, such as Dr. Alton Walker of Rockport, TX, filled in for dentists who were deployed overseas by the military. He also substituted for a dentist who had to temporarily quit working due to injuries from a car accident.</span></p></blockquote>

But there comes a point when doing so can affect the child&#8217;s bite or tooth growth. What&#8217;s the solution? Find a way to entice the child to make the move on his own. One way is to buy the pacifier. The <a target="_blank" href="http://www.timesdaily.com/article/20091130/ARTICLES/911305000/1123?Title=Bye-bye-binky" onclick="pageTracker._trackPageview('/outgoing/www.timesdaily.com/article/20091130/ARTICLES/911305000/1123?Title=Bye-bye-binky&amp;referer=');">Times Daily reports</a> on a dentist who pays his young patients to let him hang their pacifier on the wall. Fifty cents or a dollar is usually enough. <br />
<blockquote>
<p>Or, as Glinka added, &#8220;when the child is old enough to like money more than they like their pacifier.&#8221;</p>
<p>That measure of readiness is based on some 600 pacifiers that hang on the walls of their treatment room. All have come from patients who have agreed to sell them to the dentists for 50 cents apiece. Some kids bring in more than one.</p>
<p>Pero and Glinka also reward thumb- and finger-suckers for breaking the habit. The kids get $1 when they bring a calendar showing they&#8217;ve been clean for 30 days.</p>
<p>They recommend that parents step in to break the pacifier or thumb habit if it&#8217;s still going on when the child is 3 1/2 to 4. By then, it&#8217;s not serving any useful purpose and may be causing dental problems.</p>

According to the New York Times about 35% of Americans have not seen a dentist in the past year. This creates a staggering dental problem for the populace at large, which is why many dentists take it upon themselves to offer days of free dental care where possible. But the article notes these free services are only a temporary solution. The truth is everyone needs to see a dentist regular and also maintain their own dental hygiene with regular flossing and brushing.<br />

<p>The New York Times recently reported that more than 100 million people in the U.S. don’t have dental care and that roughly 35 percent of Americans haven’t seen a dentist in the last year.</p>
<p>Several dentists in Montana decided they wanted to do what they could to remedy America’s dental care problem, according to NBC. Coming from Billings, Broadview and Laurel, these dental care professionals decided to hold Give Back a Smile, a one-day free event for individuals without coverage.</p>
<p>“I don’t think anyone really enjoys coming to the dentist, but everyone we’ve had in here have just been very thankful and very grateful,” Dr Christopher Hirt told the news provider. “These are the sort of toothaches that have been keeping people up at night they can’t function, they can’t eat, and they’ve just been so grateful for us to get them to a place where they can just resume a normal life.”</p>

			<content:encoded><![CDATA[<p>Today Jim Du Molin discussed how the public views dental care and how many are resentful over the cost. To help illustrate this he referenced a recent NY Times article that discussed the growing use of dental implants. The <a target="_blank" href="http://www.nytimes.com/2009/11/17/health/17brod.html" onclick="pageTracker._trackPageview('/outgoing/www.nytimes.com/2009/11/17/health/17brod.html?referer=');">NYT article</a> said the following:<br />

<p>&#8220;In an overwhelming majority of cases, implants to replace lost teeth are by far the best long-term solution for maintaining a healthy mouth. Also, because they rarely need to be replaced, in the long run they are more economical than bridges. </p>
<p>“Bridges are not the standard of care anymore,” Dr. Lawrence J. Kessler, a periodontist and associate professor of surgery at the <a href="http://www.med.miami.edu/" onclick="pageTracker._trackPageview('/outgoing/www.med.miami.edu/?referer=');">University of Miami School of Medicine</a>, told me in an interview. “For most people who lose teeth, implants are the treatment of choice.” </p>
<p>&#8220;About half a million implants are placed each year in this country. On average, the total cost of an implant to replace a single tooth is $3,500 to $4,000 (more if other procedures are required), or about one-third more than the cost of a bridge. But while bridges have an average life span of 10 years, an implant can last a lifetime.&#8221;</p>

<p>It&#8217;s true that the procedure is expensive, in part because it is expensive for the dentist and also because it is basically a permanent fix for the patient. But as Jim points out, the interesting part of the article are the comments that followed. Many readers were resentful of the cost. Some blamed the insurance companies for treating implants as &#8220;experimental&#8221;, which is pretty absurd. Therefore, they refuse to cover the treatment. Others blamed the dentists for charging so much for the treatment.</p>
<p>Other readers defended the dentists, however. One stated: “To the person complaining about fees- you pay for the dentist’s time, skill, care and judgment, not an item off the shelf. The dentist didn’t get the skills by purchasing knowledge and skill pills off the shelf. If you are unhappy with the dentist go find one that makes you happy. Having no concept of ALL the costs incurred by the dentist, you’d be ashamed of complaining about the wonderful dentistry you received.&#8221;</p>

By incorporating EDRs, PDS affiliated owner doctors are now able to run their practices more efficiently while enhancing patient care.</p>
<p>“We are committed to providing dentists with state-of-the-art resources for their practices, and our affiliated dentists have seen a tremendous value in EDRs. Patients’ records are more precise, can be located quickly, and are backed-up to minimize the impact of catastrophic events. Furthermore, our PDS network is secure so patients can be assured that their privacy will continue to be maintained,” said Steve Thorne, Pacific Dental Services founder and president.</p>
<p>“Using EDRs has been such an advantage in running my practice. Ultimately, I was pleased that PDS offered my office the opportunity to make the transition to EDRs so that I can open a chart and study it from any workstation in my office. The time I would have spent locating my patients’ charts can now be allocated to treating the patients themselves,” said Lance Rygg, DDS. “Of course, the fact that EDRs are environmentally friendly, require less storage space, and are backed-up in the event of an emergency are also great benefits.”</p></blockquote>

The reason is to cover the cost of inflation, which grows at about that same rate every year. This means in order to be as profitable as he was last year, a dentist must raise his rates slightly. But this doesn&#8217;t happen every year. This year is a perfect example.</p>
<p>While inflation is still going to happen, a lot of dentists are foregoing the raise because of the financial strain of the recession. It&#8217;s partly out of goodwill and partly out of good business sense. If people are struggling to pay bills, they may skip going to the dentist altogether. So it makes sense to make it as easy on them as possible.</p>
